– Independently eat
– Independently use the bathroom
– Does not wander, not a secure location
– Socially isolated

Type of Program: Wellness, nursing care
Clientele Served: Adults facing life limiting or life threatening illness.
Location: Pilgrims Hospice Society, 9808- 148 street, Edmonton AB, T5N 3E8
Staff: Bonnie Ross, manager and licensed practical nurse
Fee: $8.00 per day
Hours of Operation: Monday, Tuesday and Wednesday from 9:30 to 3:30
Spaces Per Day: 10
Criteria Admission:
– Have life limiting or life threatening illness
– Must provide own transportation
– Assessment required by staff before admission

Type of Program: Rehabilitative and Wellness
Clientele Served: 18+, cognitive and physical issues

Staff: 1 Recreation Therapist (co-owner), 1 Occupational Therapist (co-owner)
Fee: $25 an hour (privately funded)
Hours of Operation: 24 hours, 7 days a week, open holidays

St. Joseph’s Day Support Program

Type of Program: Rehabilitative
Location: 10707 29th Ave, Edmonton, Alberta, T6J 6W1
Staff: 1 day program manager, 4 health care aides, 1 recreation assistant, 1 unit clerk, 1 physical therapist, and 1 occupational therapist
Fee: $8.00 per day
Hours of Operation: 1000-1600
Spaces Per Day: 30
Criteria Admission:
– Client must require rehabilitation, reassessment or maintenance
– The client must live in the Edmonton region
– The client must have and maintain contact with a physician in the community
– The client is able to function in a group
– The client does not require 1:1 and is not an elopement risk
– Client is motivated and willing to attend on a regular basis
– The client must benefit from two or more services provided by the program
– The client must not demonstrate behavior which may be harmful to themselves or others
– The client does not require a mechanical lift for transfers
– The client’s care needs can be met by the available staffing levels

St. Michael’s Day Support Program

Type of Program: Social
Clientele Served: Ages 65+ (some exceptions) for adults with physical, cognitive,and/or social limitations that are living in the community
Location: 7404 – 139th Avenue NW, Edmonton, Alberta, T5C 5H7
Staff: Director, Administrative Assistant, Licensed Practical Nurse, 3 Health Care Aides, Physical Therapist, 3 Physical Therapy Aides, Occupational Therapist, Occupational Therapy Aide
Fee: $27.50 per day – attendance up to four times weekly – same day and time
each week
Hours of Operation: Tuesday – Friday / 10:00 am – 4:00 pm
Spaces Per Day: 15
Criteria Admission:
– The Client lives in the community or in the process of being discharged into the community;
– The Client does not demonstrate behaviour which may be harmful to themself or others;
– The Client’s needs can be met by available staffing levels;
– The Client and/or family are motivated to become involved in the program;
– The Client is able to function as part of a group;
– The Client requires at least one of the following:
— a health maintenance/monitoring program
— a physical maintenance program/exercise program
— a structured leisure environment
— caregiver relief

VC Community Geriatric Psychiatry Day Programs

Type of Program: Mental Health Support
Clientele Served: Community dwelling individuals age 65 and older with an Axis I diagnosis other than neurocognitive disorders at risk for decompensation/ hospital readmission that require long term or transitional support
Location: Two locations – Hys Centre (#215, 11010 – 101 St., Edmonton, T5H4B9), and Southside/ Ermineskin (3030 – 106 St., Edmonton, T6J5M5)
Staff: Each program has one Recreation Therapist (Kristi-Hys; Janice – SS); two RTAs (Dawn, Gail – Hys; Daisy; – SS)
Fee: $8/ visit – Clients are not charged for days they do not attend; Fee reduction available
Hours of Operation: M-F 7:45am – 3:30 pm Hys; 7:30am – 3:15pm SS
Spaces Per Day: 20
Criteria Admission:
– Must have access to a reliable source of transportation to and from program
– Independent with personal care/ toileting
– No significant/ recent history of physical/verbal aggression; wandering or falls
– Ongoing treatment/ follow up with a psychiatrist and or a Community Mental Health Therapist
– Primary Axis I diagnosis other than neurocognitive disorder
– Able to tolerate ~ 5 hours of active participation in a group setting
– Continues to reside in community setting (own home, seniors apartment, group home, lodge)
Referrals: Direct referral from psychiatrists, hospital, mental health therapists/nurse using Integrated Seniors Mental Health Intake form (available on AHS Insite) or CGP Day Program Referral (available from Hys Clinic)

Young Adult Day Support Program (YADS)

Type of Program: Young Adult Day Support Program
Clientele Served: Young adults, age 18-59
Location: CapitalCare Dickinsfield, 14225 94 Street, Edmonton Alberta, T5E 6C6
Staff: Recreation Therapist, Physiotherapist, 3 Therapy Assistants, Health Care Aide, Unit Clerk
Fee: $15.72/day, Automatic Withdrawal
Hours of Operation: 9:00am – 3:00pm
Spaces Per Day: 15
Criteria Admission:
The Young Adult Day Support Program was designed for clients who:
– Can benefit from the recreation and physiotherapy programs.
– Are motivated to become involved and participate in the programs.
– Do not demonstrate behavior which may be harmful to self or others.
– Are living in the community or in the process of being discharged to the community.
– Are able to function as part of a group and offer peer support.
– Have care needs that can be met by the available staffing levels.
– Have their own physician in the community.
– Are able to administer own medication from a safely labeled package.
– Are non-smoking.

Location: First United Church, lower level, 10409 100 Avenue, Fort Saskatchewan, Alberta T8L 1Z3
Staff: activity coordinator, activity aides, volunteers
Fee: $8 per day
Hours of Operation: 9:30am to 3:30pm, Tuesdays and Thursdays
Spaces Per Day: 15 spaces on Tuesdays, 10 spaces on Thursdays
Criteria Admission:
– Are living in the community
– Have mild cognitive or physical impairment
– Have medical diagnoses and are medically stable
– Are able to look after their own personal care, or need minimum assistance (eg. with toileting, eating, medication administration)
– Are in need of a structured recreation program and are unable to access regular social opportunities in the community
– Are covered by Alberta Healthcare insurance
– Are able to function as part of a group
– Do not demonstrate behavior which may be harmful to themselves or to others
– Are manageable with the staff available

– Are able to look after their own personal care, or need minimum assistance (eg. with toileting, eating, medication administration)

Calgary Chinese Elderly Citizens Association Day Programs

Type of Program: Wellness program and dementia program
Clientele Served: Chinese seniors
Location: 111 Riverfront Avenue SW, Calgary, AB, Canada T2P 4Y8
Staff: Support Group Coordinator
Fee: Two types of programs:
1. $8 per program day
2. $10 per program day
Hours of Operation:
– Program #1: Every Monday and Friday (Wellness program) starts at 9:15 am till 3:15 pm (6 hours)
– Program #2: Every Wednesday (Dementia program) starts at 9:15 am till 3:15 pm (6 hours)
– Program #3: Every Tuesday, Wednesday and Thursday (Wellness program) starts at 10 am till 2:30 pm (4½ hours)
#1 & #2 are funded by AHS, it requires 6 hours of operation time. #3 is funded by a different funder and has a shorter operation time.

– Live in community

Father Lacombe Adult Day Program

Type of Program: Wellness Adult Day Program
Clientele Served: Home Care clients who are primarily seniors. Purpose is to provide support to allow clients to continue living as independently as possible in the community and to provide care giver respite
Location: Father Lacombe Care Centre, 270 Providence BV SE Calgary, AB T2X 0V6
Staff: RN Manager, Support LPN, HCA’s, Receptionist
Fee: $8.00 meal fee
Hours of Operation: Mon-Fri 9AM-3PM, Wed Eve 3PM-9PM
Spaces Per Day: 15/Day Mon-Fri, 10 on Wed Eve’s
Criteria Admission:
– Be registered as a Home Care Client
– Have been assessed as requiring a Wellness Program
– Be a resident of Alberta as defined in the Alberta Health Care Insurance Act
– Be living in the community or in the process of being discharged home
– Have and maintain contact with a physician in the community
– Be motivated to become involved and participate in the ADP program
– Be able to function in a group and not demonstrate behavior harmful to themselves or others
– Not require constant 1:1 supervision; if there is an elopement risk, the Client/caregiver must agree to establish a managed risk agreement or attend a secure program site: and
– Be able to attend on a regular basis
